Comprises a free smartphone application that can connect to a blood glucose monitor via Bluetooth - patients input blood glucose readings, add notes and request callbacks if needed. Clinicians use the GDm-Health desktop application to review readings and identify patients in greatest need. Regulated, CE marked and already adopted by 25 NHS Trusts. Available in UK only.
